Consuming male sperm will not affect your health in an adverse manner, provided your man is healthy and does not have any sexually transmitted diseases. You cannot get cancer by swallowing sperm. Even if your man does have any STD's, it is very unlikely that he will infect you if you consume his sperm. However, since there is a chance of that happening, you should always have protected oral sex, unless you're absolutely sure of your man's sex life.
I frequently give my boyfriend oral sex, which he obviously receives first. After that, he either falls asleep sometimes or just leaves. No, you're definitely not making a mountain out of a molehill. That's definitely not how it works with other couples, or at least it is not the way it should work.
You say "he obviously receives first" as though that is some unspoken or unwritten rule that all women have to follow. But the reality is that you have the right to receive oral sex as much as any man and also have the right to receive it first sometimes. In fact, if your boyfriend just realized the fact that you would be satisfied with oral sex and a lot more open to other sexual acts after that, he would always make sure that you go first.
Having said that, here's what you can do. Your first option is being open with your boyfriend and telling him that you'd like him to give you oral sex and that you'd like him to do it before he gets it from you. Yes, it may be difficult because your boyfriend is probably used to having his way, but you have to be honest about what you need.
Your other option is to let him receive oral sex first, but again be honest and tell him that you expect it in return too. If you're still facing the same situation even after being open about it, you'll just have to get used to the idea that your man is a "taker". Then whether you choose to live with it or not – that's your choice.
