Anembryonic Pregnancy

Whenever I got pregnant, within 2 months of my pregnancy the child growth stopped and now I have to go through D&C? Is it safe?
Dear Sumera Bashir,
Your problem is that of recurrent pregnancy loss. You seem to have an Anembryonic pregnancy (missed abortion). No, there's no need for a D&C. You can terminate the pregnancy medically with RU-486 (mifegest) and Misoprostol (prostaglandins).
I agree this is a very frustrating problem. We have found that the following empiric treatment, based on experience, helps treat many women who have experienced recurrent and early pregnancy losses.
- Metformin, 1500 mg daily.
- Folic acid, 5 mg daily.
- Low dose aspirin, 50 mg daily.
When you get pregnant, continue all the above; and also take 600 mg vaginal progesterone suppositories daily; till 20 weeks. Ask the doctor to do vaginal ultrasound scans weekly.
Urge For Baby

I am 28 yrs old and trying for a baby according to calculators since 4 months. Still I have not conceived. What could be the problem? I am worried. Could infection of any kind cause problems?
Dear Sunita,
You need to be patient - Nature takes time to make a baby! If you are not pregnant in another 9 months, then do a complete workup before starting treatment haphazardly.
You need the following simple medical tests:
- Semen analysis for your husband, to check his sperm count and motility.
- Blood tests for you for the following reproductive hormones - FSH (follicle-stimulating hormone), LH (luteinising hormone), PRL (prolactin) and TSH (thyroid stimulating hormone) on Day 3 of your cycle, (to check the quality of your eggs). Do this only from a reliable lab such as Specialty Ranbaxy.
- A HSG (hysterosalpingogram, X-ray of the uterus and tubes) on Day 8 of your cycle.
With these test results, we can determine if there is any medical problem causing your infertility. If there is a problem, then we can treat it.
Chemotherapy
Irshad asks,
I am on chemotherapy for primary left testis secondary lung cancer. My one testis is removed. So can my wife conceive? Or it is possible for us to go for insemination?
Dear Irshad,
Cancer chemotherapy destroys all rapidly multiplying cells including the spermatogenic cells, which produce sperm in the testis. Check your semen analysis. There's a very high risk that it may be zero now. You should have frozen your sperm in a sperm bank prior to starting the chemotherapy. These frozen sperm could have been used for doing ICSI for your wife.
