I have polycystic ovarian syndrome. However, I'm slim and there are no signs that I am associated with PCOS other than polycystic ovaries on ultrasound. I am not taking any treatment and I get my periods without problem. Will this condition affect my fertility in the future? I am 26 years old now. Is there a need for me to declare this condition to my future spouse? I find it difficult to broach this topic as he may assume I'm infertile.
Dear Rash,
You have what is called "lean" polycystic ovarian syndrome (PCOS). It's a common misconception that all patients with PCOS are obese, but this is not true. Yes, it's possible that you may need ovulation induction when you want to have a baby, though it's impossible to predict this. Whether or not to tell your future spouse is your personal decision - I feel it's best to tell the truth, but this does not mean that you have to volunteer information you do not want to.
Will I Get Pregnant Again?

I had a miscarriage 3 years ago. Since than I never got pregnant. I just discovered that I have PCOD. I am taking Metformin and Letrozole for last 3 months. Still no result. Do you have any advice for me?
Dear Priyanka,
Do a complete workup before starting treatment haphazardly. You need the following simple medical tests:
- Semen analysis for your husband (to check his sperm count and motility).
- Blood tests for you for the following reproductive hormones - FSH (follicle-stimulating hormone), LH ( luteinising hormone),PRL ( prolactin) and TSH ( thyroid stimulating hormone) on Day 3 of your cycle, ( to check the quality of your eggs).
- A HSG (hysterosalpingogram, X-ray of the uterus and tubes) on Day 8 of your cycle (to confirm your fallopian tubes are open).
Please send me the detailed test results and medical reports. You can scan them in as a single file and email them to me.
With these test results, we can determine what medical problems are causing your secondary infertility. The fact you have conceived once (even though you did miscarry) means your chances of having a healthy baby are excellent, so please don't get disheartened!
Late Pregnancy

I conceived via IVF and delivered a baby in Feb 08. Mine was a case of low-lying placenta. I was bleeding a lot from 5th month. Baby could not be saved as it was a Preterm delivery (7th month) and baby's weight was only 800 gms. Can I ever conceive again? I am 39!
Dear Archana,
This is truly tragic and I agree you've been extremely unlucky. Repeating the IVF cycle and transferring more embryos would be your best treatment option, as it would maximise your chances of conceiving. Time is now at a premium for you - please don't waste it! When you are 50 years old and looking back at your life, you should never have any regrets that you left any stone unturned and didn't give yourself the best shot at fertility treatment. While the outcome of any treatment is always uncertain, taking treatment at a world-class clinic can give you peace of mind you tried your best, which can be priceless!
Lack Of Ovulation

I have delayed periods since the onset of menses, the cycle got regularized after intercourse, I had two abortions and I had once taken an I-pill and wasn't married. My dates have again started fluctuating. Recently a gynaecologist suggested an ultrasound, which revealed that I am having polycystic ovary. My date was due on 5th of March and till date I have not got my periods. The pregnancy strip test is negative and moreover my boyfriend and I are not together so there are no chances of getting pregnant.

The reason you missed your periods is because you did not ovulate. This is called Anovulation. This maybe because you have PCOD (polycystic ovarian disease). You can induce a period with progesterone medication (for example, Tab Deviry, 10 mg, twice a day for 5 days). The period will start 4-6 days after taking the last tablet
