Dysmenorrheal or Menopause
Grace asks,
I am 50 yrs old. I have severe pain during my periods. One of my friends (Nurse) said this is a symptom of menopause. What is the correct age for menopause? Recently I am not getting any pain during periods. Is this a symptom for menopause?
Dear Grace,
No, this is not true. There is no connection between period pain (dysmenorrheal) and menopause. The age of menopause does vary quite a bit it can be between 45-52.
Asthenospermia - Low Sperm Motility
Parul asks,
My husband's sperm count and quality is ok but his motility is low - especially the fast moving sperms are not present. He is in on medication for last 2 months. The doctor had suggested him Androil, Clomid, Gentaplex, and an Injection called as Pregnyl. I want to know whether we are following the right medication or not.
Dear Parul,
I wish you had given me more details, such as the actual count and the percent of motility. It's hard to provide advise without this basic information. While low motility is cause for concern (this is called Asthenospermia), there is no simple correlation between sperm motility and fertility. Have you re-checked the sperm test from a reliable lab? Please do this. Don't jump to conclusions based on just one report. In any case, there's nothing we can do to improve sperm motility. If it is in fact persistently poor, then your best option would be ICSI.
IVF Donor
Donna asks,
I am 43 yrs old and I am trying to conceive. My periods are irregular. Is there a possibility of pregnancy?
Dear Donna,
I am very worried about your advanced age. This suggests you may have poor ovarian reserve. Your next step is to do a Clomid challenge test to assess your ovarian function (reserve) because you may have reached the Oopause.
Kindly do the test in following way:
- On Day 3, you need to do a blood test to measure your levels of FSH, LH, PRL and TSH. Do this from a reliable lab please - for example, Specialty Ranbaxy.
- Take 100 mg Clomiphene citrate from Day 5 - Day 9. Repeat the blood test for FSH again on Day 10; and then email me the reports.
- If the Day 3 plus Day 10 FSH levels are more than 25, this suggests you have ovarian failure, and that donor egg IVF would be your best treatment option, if you are willing to consider this.
- If it is less than 25, then consider IVF with your own eggs. While the outcome of any treatment is always uncertain, going through IVF can give you peace of mind!
